SPORTS ELIGIBILITY

Students participating in Extra-Curricular Activities will be expected to maintain high academic standards in order to be eligible to participate.  Students grades in all classes must be a C or higher to be eligible.

Each week, teachers will be expected to check current grades for athletes on their weekly travel cards.  This will be done each Friday (or last day of the week)  

Students who have a D or an F in a class will have a one-week period of Academic Probation.  During that time they must bring the grade to a C or better.  If the student has not brought the grade up by the following Friday’s grade check, they will be deemed ineligible for the remainder of the season.  One probationary week will be given to one student in each class per sports season.  If a student successfully gets his or her grade up in a class but then allows the grade to later fall below a C, then the student will become ineligible.      

Students will be designated as on Probation or Ineligible on a per-class basis.  Example:  if a student is put on Probation for Math but then later drops to a D in Social Studies, the prior probation has no bearing on the new low grade.
Sublimity Athletic Rules
1.  All school rules apply

2.  Students are to ride the bus to and from events UNLESS they have written permission from a      parent to ride with someone else or UNLESS their parent checks in personally with the               coach to take them home directly from a meet.

3.  Students are responsible for their uniforms. The athlete will replace any lost uniforms.

4.  Students should make clear arrangements for transportation home after practice and not            hang around school.

5.  Some games and practices are not directly after school, so students may not leave school        grounds unless they have a parent permission slip excusing him/her for an appointment.              Students are to stay on campus and study or read before the practice or game; if another             team is playing in a game, they should be at the game supporting that team.

6.  All athletes are required to have a signed athletic permission form prior to practice or competitions.  
                        
7.  All athletes are required to have a valid sports physical prior to practice or competition.             One sports physical is all that is necessary unless the health or physical condition of the              athlete changes; a new physical or doctor’s release is then necessary before the athlete may        practice or compete. Forms are available at the middle school office.

8.  Be covered by health insurance; if needed, student accident insurance is available from the         school.

9.  All activity fees must be paid or arrangements made for assistance before athlete may                 participate.

10. If students receive a citation they will miss the next contest.

11. If students receive a referral they will miss a minimum of the next contest with the athletic director and coaches input on any further suspensions.

SUBLIMITY SCHOOL ATHLETIC PARTICIPATION AGREEMENT

I have read, understand and agree to the rules and regulations set forth in the Athletic Participation Agreement.  In addition I have read, and I agree to the following rules and consequences for violations of these rules:
SECTION I.

I WILL NOT
1.  Use or possess alcohol in any form.
2.  Use or possess tobacco in any form.
3.  Use or possess illegal drugs or narcotics.
4.  Be involved in criminal activity, including any actions relating to theft, vandalism, or destruction of property.

CONSEQUENCES:
Immediate removal from the team for the remainder of the season.  (The athletic director or principal may extend this to one or more sport seasons as decided).
SECTION II.

I WILL NOT:
1.  Be involved in a fight, use threats or threatening actions against others, use or         possess dangerous items which may be considered weapons.
2.  Use profane or obscene language/gestures directed at school employees, coaches, fans, other players, or team officials.
3.  Use any actions or words considered insubordinate to coaches.
4.  Use any actions or words that bring discredit to the team or Sublimity including unsportsmanlike behavior.

CONSEQUENCES:  
1.  First violation will result in an immediate one-week suspension from participation.
2.  Second violation will result in removal from the team for the remainder of the season.
    (This may be extended to one or more sport seasons as decided by the athletic director or principal.)
3.  Further discipline action may be taken by the Administration as deemed necessary.




SECTION III.
PLAYER RULES
1.  Attend all practices unless for properly excused reason.
A.  Absent from school (sick or parental excuse) is an excused absence.
B.  Prearranged absence (the coach is contacted in advance and excuses the                                       absence.)  
C.  If an athlete misses the practices on the day before a game, playing time may be reduced.  NOT excused: In School or Out of School Suspensions; after school detentions to Finish class work or make up class time; Referral.
    
    
2.  Athletes are to display proper behavior during practice.  Practice is actually a                             classroom where the individual is expected to act accordingly.

3.  Athlete is expected to conduct him/herself as a respectable young adult during all contest: displaying good sportsmanship and courtesies.  This also includes the bus ride to and from events.

CONSEQUENCES:
1ST Offense-   Suspension for 1 game
2ND Offense- Suspension for 1 week
3RD Offense- Removal from the team

SECTION IV
Conditioning and Training:

OSAA regulations covering conditioning and training will be followed.  All participants are to report injuries to the coach as soon as possible.  The coach will fill out an accident form and submit it to the school office.  All athletes will have a minimum of two weeks of practice before participation in an event.  (An exception can occur if the athlete is going from one sport season to the next and there are not enough days to accomplish the two weeks of practice.)

Absence During School Day
Athletes must attend school for at least ½ of the day of a game (12:40- 3:00); to be allowed to participate.  If the absence is for an appointment or planned reason, the coach or athletic director should be informed 24 hours in advance.

In-School or Out of School Suspensions:

An athlete placed in Suspension (in-school or out-of-school) for disciplinary reasons will not be allowed to participate in the next available game following the suspension.  A second suspension during that season will result in removal from the team.

My son/daughter has my permission to compete in the above named sport(s) and to go with the coach on any regularly scheduled trips.  While I expect school authorities to exercise reasonable precautions to avoid injury, I understand that there is a risk of injury in any athletic activity, and that the school assumes no financial obligation for any injury that may occur.  I must present a doctor’s release when my child returns to participation after an injury.  I am advised that students are held responsible for all players’ equipment owned and issued by the school.  I give permission for my son’s/daughter’s name to be published in articles given to the media or general public regarding the above-mentioned activity.

INSURANCE ARRANGEMENTS

Some form of health insurance must cover all students.  Please check the following:
